Do horses eventually die of old age? If so, how old do they get?

Horses start losing health gradually at 25 years old. With proper care, they will eventually live to 32-33 without using any Black Market items.

The item Black Orchid resets your horse's health, morale, and energy to 100%. You can use this to extend an old horse's lifespan, but its health will continue to go down each time it is aged.
The item Philosopher's Stone makes a horse immortal. Immortal horses cannot die, and will not lose health after 25 years.

When they get 25 years old, their health begins to deteriorate.
With good care, lots of them can get to 31 or 32 years old before they die.

When their health is close to zero, you can give them a BMI called Black Orchid, which will restore health to 100%. From that moment on, health will start deteriorating again. You can give them a Black Orchid as many times as you want, and this way horses can get very very old.

Giving them a Philosopher's Stone will make them immortal, in that case their health will not deteriorate and they will never die.

My horses regularly live up to 36-38+ years old, and that's with putting them in missions and competitions every day and keeping their energy above 20%. Occasionally, missions will bring their health down a percentage point, and if you enter them into the Grand Prix, their health certainly takes a hit, but if you keep your horses' energy above 20%, their health will only decrease about 1% a day from age 25.
